Either way this wreaks havoc on the air fuel. Also may want to check the fuel injector wires individually with a noid light may need to also get fuel pressure checked.
My 2005 Toyota tundra 47 has a surge type hesitation only at 2000 to 3000 rpm Ive changed the fuel pressure regulator and spark plugs its bout to drive me crazy. When it dries out the fuel tank cant hold pressure anymore. Insert the t-fitting from the fuel-pressure testing kit into the fuel-supply line on one side and the fuel-injection rail on the other.
Fuel trims are definitely high on LTFT 1 and 2. Its not really a quick connect since it requires a special tool or at least a couple picks or small flathead screwdrivers to get it to release its actually kind of a pain in the ass to. Now it will stay.

Since you have the fuel tester do a fuel test and is low readings replace filter and do the test again Fuel pressure regulator Fuel pressure at no vacuum 265 - 304 kPa 27 - 31 kgfcm2 38 - 44 psi no vacuum 226 - 265 kPa 23 - 27 kgfcm2 33 - 38 psi Fuel pump Resistance at 20 C 68 F 02 - 30 Г.
B Diesel fuel economy ratings could be incredible - a diesel half-ton Tundra might get 23mpg combined if paired with a new 8 speed automatic. With the ignition switch ON and the fuel pump running the fuel pressure indicated by the fuel pressure gage should be 333-376 kPa 48-55 psi. A Typical Fuel Pressure Regulator.
May want to try a power balance test with your fuel injectors unplug each one one at a time from the electrical harness kinda the same way you did with the COPS. I would recheck all your vacuum lines that you may have removed you replaced the plugs. Its on the left side of the cylinder head right below where the PCV valve is if I remember right.
